1 of 3: You get this quest by looting the crazed loner in the Sundermount region. See the Sundermount tour for info on how to get to him.
2 of 3: Go to the Gallows region on the Kirkwall Day map and find Azure Jamos near the NW corner.
3 of 3: Talk to him to hand the eyes over and get 1 gold coin and 750 XP.
