How do I use an inventory item?
There are two things that you can do with an inventory item -- use it directly on something in the game world, or combine it with something else in your inventory.
Either way, start by pressing TAB to enter your inventory (or moving up on a gamepad's d-pad). Move left or right using the A and D keys or the left and right arrows on a gamepad's d-pad to find the item you want to use, and then click on the item or move up on a d-pad to select it.
If you want to use the item on something else in the game world, you will first need to be standing close to that other item, or be in focus mode and aimed at the item. Then, if the inventory item is selected, you should see a "gears" icon above the item in inventory. Either click on the gears icon or press up on your d-pad again, and you will use the item. (You may also see an appropriate icon in the lower-right corner of the screen, as well, but only if you've already selected the inventory item. That icon will do the same thing as the "gears".)
If you want to combine two items, first start by selecting the item as before. Once the first item that you want to combine is selected, select the wrench icon on the right of the item -- click on it with your mouse, or d-pad to the right if you have a gamepad. The item you just chose will remain "highlighted" above the inventory, but you will now be able to find a second item by scrolling left and right in your inventory. Pressing up on this second item to select it will have the game attempt to combine it with the first.
Also, in a few rare cases, you may still be able to move your character once an item is selected in inventory, but this is not entirely consistent. (This generally only seems to be permitted when you have an item that needs to be used from a distance.)
