1 of 4: It's in the Z-43 Innovative Toxins Plant (location 9 on the Big MT Map).
2 of 4: Enter the building and go down the big ramp, then go across some steps to reach a doorway on your left.
3 of 4: Kill the freaky harness guys who attack you, then go through the doorway and turn left.
4 of 4: The holotape with the code you need is on the floor next to some filing cabinets.
