2 of 4: Move up the stairs and toward the door.
3 of 4: Then go down the hallway until the door at the end opens.
4 of 4: Quickly shoot the Nightmares that fly out through the door while backing away -- like the ones in visions before, these will hurt you if they can explode next to you.
