2 of 3: To get this achievement, you'll need to survive the Combine ambush at the White Forest Inn in Chapter 5's Section 2: White Forest Inn.
3 of 3: Specifically, the achievement is awarded right after you knock over the turret in the force field generator building (provided you've already killed all the soldiers and hunters).
