What happened to Kasumi?
From what I gathered from conversations and letters, Takae asked Kasumi to clean the older of the two baths in the ryokan, and she slipped and fell into the murky water, where she drowned because she couldn't climb back out. Maybe she hit her head or something too. Pretty horrible, especially since Takae forced the situation to keep her from getting away from the ryokan -- and by God, it worked.
