Getting this ending is a matter of paying lots of attention to Maria and playing down your grief about Mary.
To forget about Mary:
-- Don't look at Mary's photo or letter in inventory.
-- Don't go back to Neely's Bar after you leave the hospital.
-- Don't listen to the conversation in the last hallway all the way to the end; run to get to the last door before it finishes.
To fall for Maria:
-- Walk instead of running when Maria is with you. Try not to bump into her if you can.
-- Don't let the demons hurt her much. (She will always take some damage in the hospital basement while you're being chased by Pyramid Head; this doesn't seem to change the ending.)
-- After Maria takes to her bed in the hospital, check on her several times and talk to her when you do.
-- After you find the live Maria in the labyrinth, come back to her a couple of times to see how she's doing.
And finally:
-- Keep your health high and don't look at Angela's knife in inventory. This keeps you from tipping over into In-Water.
