Specifics
This is the hardest of the four regular endings to get. Sometimes there's a magazine article next to the sewing machine in Apartment 205 of the Wood Side Apartment (where you get the flashlight) with an article about suicide. It speaks of dwelling on the past, planning how to die, doing stupid dangerous things, inviting disaster, not talking to people, and letting friendships grow distant. Here's how:
-- Dwelling on the past: Look at Mary's photo and letter several times.
-- Planning how to die: Look at the knife you took from Angela a couple of times (presumably contemplating suicide).
-- Doing stupid dangerous things, inviting disaster: Let your health get down to the flashing red cross and leave it there for the entire game. If you're playing at a harder level, you'll have to gauge this carefully so you don't get killed every time you bump into a wall. You might find it easier to survive if you run away rather than fighting when possible.
-- Not talking to people, letting friendships grow distant: Don't pay attention to Maria or visit her in the hospital. Don't come back to her jail cell after you leave it. Don't worry too much about her getting hurt (in fact, if you run into her full tilt she makes a very satisfying "oof!").
In addition, the following things have been recommended. I don't know how much effect they have, but they can't hurt.
-- Read the diary on the roof of the hospital several times (because it seems to describe a suicide).
-- Listen to the headphones in Reverse Lakeview Hotel (reliving a bad memory).
-- Listen to the conversation in the last hallway before the Big Boss all the way through (more bad memories).
