Deformed Swamp Tentacle
1 of 5: There are five of these scattered about the outdoor world area.
2 of 5: Two of them are on a huge rock that's next to the outer wall of the Fringe, right near the wall's far eastern corner. Unfortunately, they're ridiculously hard to get to. (You could jump over to where you can grab one by getting up on the huge, twisting root to the south, but it's much easier to just find one of the other plants listed below.)
3 of 5: Around on the west side of the Fringe, go to a spot that's almost directly west of the world map marker for the Door to Cyrodiil and look for a very small peninsula which has a huge mushroom tree on it and a big rock sticking out over the water. On top of the rock will be a small grummite totem carving. The Deformed Swamp Tentacle plant that you want is attached to that rock, down near the water.
4 of 5: There's one on the side of a large boulder that's next to the path a very short distance SW of Lost Time Camp (location 18 on the world map). Note that there are two more Swamp Tentacle plants on the rock on the other side of the path, but they only give regular tentacles.
5 of 5: Walk down the path that leads SE from Pinnacle Rock (location 20) until you see a large boulder to the left of the path on some higher ground. The Deformed Swamp Tentacle plant is on that boulder's south side.
