The Elder Scrolls V: Skyrim Hints
Kill Cicero, or leave the Sanctuary.
1 of 3: Either attack Cicero to start a tough fight with him (he's not totally helpless, even though he's injured) or turn around and leave without attacking him.
2 of 3: Either way has the same result as far as the main-line quests are concerned. The only difference is that you can get Cicero's unique outfit if you kill him, and he can be a follower later on if you don't.
3 of 3: NOTE: You'll get the next objective after you either kill Cicero or leave the sanctuary without attacking him.
