Kill Grisvar the Unlucky.
1 of 5: If you don't have a shiv, mention that fact to Madanach to get one.
2 of 5: You'll then need to decide whether to aid Madanach in his escape or kill him and use his escape plan for yourself. Aiding him will get you the Old Gods armor set (see the Unique Light Armor List) as a reward, and killing him will get you the Silver-Blood Family Ring (see the Unique Rings List) instead. Also note that aiding him requires much less combat than killing him.
3 of 5: If you want to kill Madanach, sneak-attack him with a shiv to get the "Kill Madanach" objective. Otherwise, continue with this objective.
4 of 5: Go find Grisvar the Unlucky is his usual area, then talk to him and say that Madanach says hello.
5 of 5: He'll turn hostile, but you don't have to fight him yourself. Just dodge his blows (if necessary) until the other mine residents finish him off for you.
