1 of 5: Return to the White Phial and tell Quintus that you have the materials he requested.
2 of 5: Follow him along until he talks to you, at which time he'll ask you how you want the White Phial attuned.
3 of 5: You'll have six choices. Which one you choose will determine what type of potion will appear in the White Phial once every 24 hours.
4 of 5: The options, in top-to-bottom order, are: Restore Health, Resist Magic, Fortify Stamina, Fortify Magicka, Fortify One-Handed, Fortify Sneak.
5 of 5: You'll find the White Phial in the "potions" section of your inventory, and it'll start out full. You can use it to drink its contents, after which it'll be 24 hours before it refills with the same type of potion.
