I just hopped of the elevator and died for no reason. What happened?
1 of 2: Quite often people do not realize that the elevator in which the civilian drops out of goes back up after a few seconds.
2 of 2: Occasionally the civilian gets caught between the moving elevator and the ceiling and dies because of it. Be careful to jump out of the elevator relatively quickly to avoid this simple error.
