1 of 4: After your character automatically uses his witcher's amulet, follow the floating purple-energy skull along until it vanishes.
2 of 4: Hit the "Use Medallion" key to get it to reappear, then follow it along again.
3 of 4: Keep doing that and avoiding combat with knight wraiths until your guiding skull hits the green-mist barrier in a certain spot.
4 of 4: Push into the barrier at that spot to exit the spectral battlefield, then follow Zoltan to where a group of elves talk to you.
