Talk to Letho.
1 of 4: Run over to Letho and click to talk to him.
2 of 4: When the "let's finish this" option comes up, you can use it to quickly end the conversation with a choice: either fight Letho to the death or let him go.
3 of 4: If you choose to question Letho first, you can find out a lot of interesting things about Geralt's forgotten past before having to decide what to do with him.
4 of 4: If you decide to fight him, you'll get the "Defeat Letho" objective. Otherwise, you'll get the game-ending cut-scenes right away.
