1 of 4: You can get one from Moira Brown as a reward for one of the objectives of the "The Wasteland Survival Guide" misc. quest.
2 of 4: In the Little Lamplight caverns (location 11 on the Zone 4 Map), the kid named Knick Knack in the souvenir shop sells one. (See the "Rescue from Paradise" misc. quest for info on getting access to the caverns.) You can either buy it from him, or pickpocket his supply key and then steal it from his shop's supply cabinet.
3 of 4: There's one on the work bench inside the shack at Jocko's Pop & Gas Stop (location 6 on the Zone 7 Map).
4 of 4: Another one is in Tenpenny Tower (location 13 on the Zone 7 Map), inside the safe in the western room in the Tenpenny Suites area. To get the safe's key, you'll have to either kill Herbert "Daring" Dashwood or do the "The Missing Manservant" undocumented quest.
