Kill Orchendor.
1 of 4: Choose the last dialogue option to exit the conversation with Peryite, then travel to Bthardamz, which is a ways west of the shrine.
2 of 4: Kill the afflicted guys who attack you as you make your way through the exterior ruins to a spot with a raised spear gate.
3 of 4: Use the lever to the right of the spears to lower them, then go past them and through a door to find the entrance to the Bthardamz Upper District area.
4 of 4: You'll need to make your way through that and four other areas to find Orchendor.
