1 of 2: Follow the quest arrow to Dr. Richards and tell him Polatli sent you to help.
2 of 2: When he asks if you've had medical experience, tell him you have (requires Medicine 20+ or a karma-reducing lie) to get the "Help Dr. Richards with his patients" objective, or say no to jump straight to the "Return to Major Polatli" objective.
